Transition plans for children with Statements of Special Educational Needs

For children with Statements of Special Educational Needs, the Code of Practice (DfES, 2001) requires that a Transition Plan is prepared following the Year 9 annual review. The focus of the Transition Plan is on helping the child prepare for a successful transition into adult life.

 

The procedures for the transition to Key Stage 4 for children with Statements of Special Educational Need are clearly defined but it is important to recognize that there will also be significant numbers of children without statements whose transitions to Key Stage 4 and beyond require careful consideration. The principles and many of the processes outlined (DfES, 2001a, 2001b) for effective transition planning are also relevant to these children.
