The blood–brain barrier restricts the entry of microbes and drugs from the capillaries into the brain due to the strong tight junctions between the endothelial cells. Bacteria and viruses have difficulty passing through uninflamed endothelium but inflammation weakens the tight junctions and allows passage of microbes into the brain. Similarly, drugs like penicillin pass more easily through inflamed capillaries than through uninflamed vasculature. In addition, lipophilic drugs pass through the blood–brain barrier more easily than do charged hydrophilic drugs. Molecules required for normal brain function, such as glucose, have specific transporters that allow passage into the brain.
